Intramuscular Injection (Back) VR Nursing Simulation
This scenario covers the ventrogluteal and dorsogluteal sites for larger-volume or viscous intramuscular medications. The learner positions the patient, landmarks the site using the greater trochanter and iliac crest, and performs a Z-track injection to reduce leakage and irritation.
Learning objectives
By the end of this experience the learner can:
- Position the patient for safe posterior intramuscular injection.
- Landmark the ventrogluteal site using the hand-placement method.
- Perform a Z-track injection correctly.
- Avoid the sciatic nerve and major vessels.

Procedure steps
- Position the patient side-lying with the upper knee flexed.
- Place the heel of the hand on the greater trochanter and identify the V between the fingers.
- Displace the skin laterally, insert at 90° and inject slowly.
- Hold for ten seconds, withdraw and release the skin to seal the track.
- Check the site and document the administration.
Assessment
The scenario flags any insertion outside the safe zone and records whether the Z-track was maintained.
Equipment and setting
Standalone VR headset with hand controllers, full-scale virtual patient and equipment, instructor screen mirroring, Arabic and English interface, offline operation on the campus network.
