Intramuscular Injection (Shoulder) VR Nursing Simulation
The learner administers an intramuscular injection into the deltoid of a virtual adult patient. The scenario emphasises landmarking two to three finger-widths below the acromion, a 90° insertion, the maximum deltoid volume, and monitoring the patient afterwards.
Learning objectives
By the end of this experience the learner can:
- Landmark the deltoid injection site correctly.
- Insert at 90° with the appropriate needle length and depth.
- Inject slowly and withdraw with correct pressure and site care.
- Recognise contraindications for the deltoid route.

Procedure steps
- Expose the shoulder and identify the acromion process.
- Mark the injection site in the centre of the deltoid triangle.
- Clean the site, stretch the skin and insert the needle at 90°.
- Inject slowly, withdraw, apply gentle pressure and dispose of the sharp.
- Observe the patient and document the site and response.
Assessment
Site accuracy, needle angle and injection speed are scored against a validated checklist.
Equipment and setting
Standalone VR headset with hand controllers, full-scale virtual patient and equipment, instructor screen mirroring, Arabic and English interface, offline operation on the campus network.
