Anaphylaxis VR Nursing Simulation
The virtual patient develops anaphylaxis minutes after an intravenous antibiotic: stridor, wheeze, hypotension and a spreading rash. The learner must recognise it, stop the trigger, call for help, administer intramuscular adrenaline in the correct dose and site, position the patient, give oxygen and fluids, and monitor for a biphasic reaction.
Learning objectives
By the end of this experience the learner can:
- Recognise the airway, breathing, circulation and skin signs of anaphylaxis.
- Stop the trigger and call the emergency team.
- Give intramuscular adrenaline into the anterolateral thigh at the correct dose.
- Position, oxygenate, give fluids and monitor for recurrence.

Procedure steps
- Stop the infusion and assess airway, breathing and circulation.
- Call for help and the emergency trolley.
- Administer adrenaline 0.5 mg IM and note the time.
- Lay the patient flat with legs raised, give high-flow oxygen and IV fluids.
- Repeat adrenaline after five minutes if no improvement and document everything.
Assessment
Time to adrenaline, dose and route accuracy and the monitoring plan are scored.
Equipment and setting
Standalone VR headset with hand controllers, full-scale virtual patient and equipment, instructor screen mirroring, Arabic and English interface, offline operation on the campus network.
